Man killed, hospital evacuated
Jocelyn BELL
Torstar News Service -- page A32
An industrial accident at a Niagara Falls chemical plant killed one man and forced a hospital to evacuate its emergency room.
Charles CONDEZ and another employee at Mancuso Chemicals Ltd. were pouring two 500-kilogram bags of formaldehyde powder into a mixture of phenol and water at about 11: 20 a.m. yesterday.
After the second bag of formaldehyde was added to the mix, the men noticed an unusual chemical reaction. Fearing the mixture was about to explode, they tried to run. But it erupted before 31-year-old CONDEZ could escape, and he was splashed with the toxic substance.
His co-worker ran to get help from other workers. They carried the unconscious CONDEZ from the area, took his clothes off and sprayed him with water, trying to decontaminate him. They also performed cardio-pulmonary resuscitation.
Firefighters and paramedics carried the man to an ambulance. Although firefighters were wearing blue medical gloves, the chemical residue on CONDEZ's body corroded the rubber and burned the hands of three firefighters. They and about four Mancuso employees were treated for burns at Greater Niagara General Hospital.
CONDEZ was taken to the same hospital and pronounced dead. The toxic fumes emanating from his body were so strong that nurses had to borrow breathing masks from firefighters.
The fumes permeated the emergency waiting room and the examination rooms, where about seven patients were already admitted. It also got into the ventilation system, and blew into several operating rooms.
Hospital officials evacuated the emergency and operating rooms. All patients were driven to other hospitals.
The ministry of labour is investigating CONDEZ's death.

BLUMENFELD, Sam -- 1926 - 2002. Following a lengthy and courageous battle, Sam BLUMENFELD passed away peacefully, with his family at his side, on December 8, 2002, in Richmond Hill, Ontario. He is survived by his wife of more than 55 years, Sara, and their children Dalia, Ray, and Miriam, his grandchildren Zachary, Jessica, and Ben, his sisters and brother with their spouses, Bianca and Frank BLANARU, Clara and Jack OREN, Sara and Sandu GOODMAN, and Yitzchak and Malvena BLUMENFELD, his brother-in-law Rubin and Clara MAYER, the MAYER family, the MANEL family, the HABERMAN family, and the MIZRACHI families. Sam and Sara immigrated to Canada in 1953 with limited resources and learned to adapt to a new land, a new culture and a new language. He purchased Niagara Esso Service in 1957 and ran this business until his retirement in 1988. Over more than 30 years of serving his customers in Winnipeg, Sam built a wide network of Friends and associates. He was proud of the success of his business and was a leader in supporting the rights of his industry. Although his beginnings were humble, he was self-educated and well versed, with a curious mind, strong will and many interests. Our family wishes to offer our gratitude to the staff of York Central Hospital in Richmond Hill, Ontario with special thanks to Brenda HRYSIO and her entire team of men and women, the staff on the third floor of the Continuing Care Centre, Donna BELMORE, Dr. ZATSMAN and in particular Dr. FITURI. Their care, compassion and concern for Sam is truly appreciated. Donations can be made to the Crohn's and Colitis Foundation of Canada.
